Direction: The office is located on Sunbridge Dr. between N College Ave and N Gregg Ave. From Springdale/Rogers/Bentonville area: Head south on I-540 take exit 66 for AR-112. Turn right on AR-112 S/Garland Ave then turn left on W Drake St. Next, turn right onto N Gregg Ave then left on Sunbridge Dr. The office will be on the left. From the University of Arkansas: Head west on W Maple St. Then turn right on the Garland Ave. Next, turn right onto W North St. Then turn left on N Gregg Ave and turn right on Sunbridge Dr. The office will be on the left. From the Northwest Arkansas Mall: Head south on N College Ave. Then turn right onto Sunbridge Dr. The office will be on your right. When you arrive When you arrive, enter from the main entrance of the building and go straight through the hallway. The waiting room will be in front of you on the left. Please make yourself comfortable and I will be right with you at your scheduled time.

Insurance: James R. Parks, M.D. PLLC is in network for several major insurance companies. Please contact your insurance company to check eligibility or you may contact our office for assistance. Unfortunately, we do not accept Medicare/Medicaid.
Cancellation/No-show policy: We require a minimum of 48 business-hours to cancel a new patient evaluation and a minimum 24 business-hours to cancel a follow-up appointment. Furthermore, a fee will be assessed for missed appointment, late appointment, and late cancellation. Please refer to our office policy and procedure for more information.

Leave a message including your name, contact number(s), best time to call, and reason for calling and I will make every effort to return your call the same day. Messages left late in the day or on weekends/holidays may not be returned until the following business day. However, I may not reply to e-mails as promptly as voicemails. Please be aware that email is not a confidential form of communication, and is not appropriate in a medical emergency.
